C2A Community Connections encourages and supports you to actively get involved through a range of tailored programs created to provide learning opportunities and memorable experiences that are right for you and your family.

Our dedicated and qualified staff are here to deliver daytime group-based activities and individual support services that are designed to connect you with your community in a fun, friendly and inclusive way. Make new friends, learn new skills, have some fun, and enjoy activities that focus on your physical health and emotional wellbeing.

We’ll help you find the services that are right for you and get the most our of every day.


Community hubs

It’s your life so what would you like to do? What activities would make you happy?

Community Connections is available from four C2A hubs located in Frankston, Mornington, Wonthaggi and Traralgon.

Each C2A hub has a range of different activities for you to choose from that have been developed to help establish positive social connections and build individual capacity through life skills training and development in a fun learning environment.

Group programs and individual support is available Monday to Friday, 9am-3pm (weekends and public holidays are also available by arrangement) and are based around the Key Learning Goals of Health/Fitness, Development, Independent Living, and Education.

  • Do volunteer work
  • Attend appointments
  • Learn living skills
  • Go to community events
  • Learn new hobbies and activities
  • Be a sports fan
  • Go to the theatre
  • Participate in music and theatre performances
  • Learn how to cook
  • Get travel training and support
  • Go on shopping trips
  • Express yourself through arts, craft and music
  • Enjoy swimming
  • Riding for the Disabled (just to name a few)

C2A Community Connections acknowledges families may also need support, resources and information from time to time so care for the carer is at the forefront of all we do.
